Hmmm...Maybe it's My Promise to You be Moonshadow Woman. Here's a summary:

Justin goes to NY after Rage Party. While crossing the street he gets hit by a car and has significant damage to his face. He is taken in by a wealthy older man who pays for reconstructive surgery. Justin and the man move to Europe. Years later Brian just happens to be in Europe and sees Justin. While Justin looks nothing like he did before, Brian is convinced it is him.
