Justin returns to Pitts and he and brian has secret relationship. Just is hiding that he is sick

Please help? I am looking for story where Justin and Brian break up and Justin leaves town. He returns later and he and Brian start a secret relationship. Justin is hiding that he is sick from everyone including Brian. He writes notes to himself to help him remember things. Brian wants the gang to know they are back together but Justin does not. I remember a scene where Justin tries to set  Brian up on a date with a friend of his and Brian get's upset. Also, remember Justin telling Brian not to fall in love with him. I checked tags for secret relationships and Justin's ill but cannot find it. It is driving me crazy.
