Can't remember the name or author of this story. Brian is jealous at first over hearing a conversation between Justin adn Alex (B not knowing it's J's sister and of course assuming the worst.)
Can't find it in the tags. If it's there, I obviously don't know what to look under! LOL

I couldn't find a story like this where Alex is Justin's sister but I did find a story that fits your request where Alex is a female friend of Justin's. Everything fits the request except the sister part. It is a short story/stand alone. Hopefully it is the one you are looking for. Here is a little paragraph from the story:

"Listen Alex, I have to hang up. Brian must be about to arrive and I don’t want him to know." The young man was saying. Brian felt a chill down his back. What was that he shouldn’t know? Justin laughed again and what he said then, left Brian cold. "Yes, I’ll see you tomorrow, but I don’t know if I’ll have enough strength. Every time you leave me more exhausted and those positions that you force me to do! Wow, Alex, the truth is that I thought I would no longer be able to bend like that" And he laughed again as he hung the phone.

Thanks, but that's not it. In the one I'm looking for Alex was definitely his sister. She'd been kicked out of the house as she was a lesbian. No contact with Craig or Jennifer since.
She'd been in the hospital in NY for a couple of years due to an accident and mishaps while in the hospital.
I think I remember her telling Brian about how she's exacting revenge on Craig with IRS...
