Brian misunderstanding...

I'm looking for a particular fic, and wondering if its been updated.

I think its called Heart but I can't remember.

It consist of Brian thinking that Justin died in the hospital, and takes off for the west coast on a motorcycle. Over the course of a year (I think) he comes to understand how he loved Justin, and begins changing with the help of new friends on a ranch?

Meanwhile back at the hospital Justin is waking up from the coma asking for Brian and no one knows where he is. They know he thinks Justin has died. Justin begins to recovery determined that when he finds Brian he will be stronger? I think a certain a mount of time passes and Brian by an Article of a friend of his, shows Justin Taylor alive. He's in such shock he can't even move.... I think the last part has him talking on the phone with Justin?

Can anyone help me out?
